About Us

Kathy Fagan is a native of Nashville, TN, where she attended and graduated from Tennessee’s oldest public school, Hume-Fogg Academic High School. Kathy graduated from University of Evansville with her Masters in Physical Therapy (PT) in 1996 . Since then, she has worked in several capacities and environments within the PT world. She started out at Greenview Hospital in Bowling Green, KY, as a staff Physical Therapist. Within six months, she was offered a therapist staffing position in Lexington, KY. After a year in Lexington, Mike joined the Marine Corps, and the family was relocated to Camp LeJeune, NC. Kathy started an outpatient clinic from the ground up in Jacksonville, NC, and it wasn’t long before it was very successful!

2001 saw the family looking to move again, this time to the Nashville area, as Mike was getting out of the Marines and into law enforcement.  Since then, Kathy has worked in the home health setting covering just about all of Middle Tennessee. Kathy has earned the distinction as a Certified Geriatric Doula (EOLD), a Functional Strategies for Optimal Aging Expert (FSOAE), a Geriatric Trained and Certified Clinical Specialist (GTCCS) as well as a board certified Geriatric Certified Specialist (GCS).  There are currently only fifty-five Geriatric Trained and Certified Clinical Specialists practicing in the USA.

Mike has worked in Home Health since 2009 as a Physical Therapy Assistant (PTA) and has earned distinctions both as a Functional Strategies for Optimal Aging Expert (FSOAE) and a Certified Geriatric Doula (EOLD).

Currently, Fagan Geriatric Wellness & Rehabilitation is one of the only outpatient clinics with these specializations in geriatrics. While providing evidence-based evaluation and treatment of movement and functional deficits, practitioners intervene towards overall wellness in aging development, isolation management, integrity in life review, and more.

                                                                                                                                                            

ABOUT OUR PRACTICE
At Fagan Geriatric Wellness and Rehabilitation, we have over 30 years of clinical practice combined with specialized training in an evidence-based approach to wellness and rehabilitation in the geriatric community.

Office Hours
While appointments in the clinic or therapeutic visits may be scheduled outside of normal business hours,
the office of Fagan Geriatric Wellness and Rehabilitation is open Monday through Friday, 8:00am to 5:00pm.

Location
Our office is located inside The Manor of Gallatin at 143 Prince Street in Gallatin, TN.  Parking is available at the side of the building, and the front entrance is wheelchair accessible.

Payments and Insurance
We accept Medicare and many commercial insurance programs, in addition to cash, check, VISA and Mastercard.
